Item 5.02 Departure of Directors or Certain Officers; Election of Directors; Appointment of Certain Officers; Compensatory Arrangements of Certain Officers.
 
On June 19, 2019, Surgery Partners, Inc. (the “Company”) announced that Angela Justice, Ph.D., the Executive Vice President and Chief Human Resources Officer of the Company, will be leaving the Company effective July 1, 2019. Dr. Justice will continue with the Company in a consulting capacity through July 30, 2019.
